Hello everyone! Here are the Camp Misarvik registration form and medical form in image format.

👉 You can save the photos and print them.
👉 Once the registration form is completed, you can send it back to us:

by private message on this page (photos)
or by email at: [email protected] (scans or photos)

📋 To complete your registration, the medical form must also be filled out:

by a healthcare professional (doctor or nurse)
and sent to us before end of May

👉 Both forms are included directly in this post.

🗣️The Inuktitut version of the registration form is also included.
