Researchers ran MRIs on 45 people with zero hip pain, aged 15-66. Here's what they found…

69% had labral tears
24% had chondral defects
22% had fibrocystic changes at the head/neck junction
20% had osseous bumps
13% had labral or paralabral cysts
11% had acetabular bone edema
11% had rim fractures

All of that in hips that felt fine.

So when a client walks in with a scan that says "labral tear," is the tear the reason they hurt? Maybe. Maybe not. Plenty of people are walking around with the exact same finding and no pain at all.

A tear on a scan is not a pain diagnosis. It's one piece of information. The nervous system, the surrounding tissue, how the person moves and what they've been told about their body all play a role in what they feel.

That's good news for your clients. A structural finding doesn't automatically mean a life sentence of pain. And it's good news for you, because there's often a lot more to work with than the picture suggests.

Treat the person in front of you, not the MRI. Watch for change, and see what shifts.

Why do we make clients move during their treatment? 🤔

Spoiler…it's not to stretch anything.

Your hands create an opportunity. Movement is how the nervous system actually uses it. 🙌

New sensory information, fluid exchange, confidence, and sensorimotor learning, and a whole lot more that's seriously cool.

Pain at the outside of the elbow.

What is it?

Tennis elbow??? Maybe. But let's look at a few other things that it could be.

-Lateral epicondylalgia-Common extensor tendinopathy-Partial tear of the common extensor tendon-Radial tunnel syndrome-Posterior interosseous nerve entrapment-Radiocapitellar joint irritation-Posterolateral synovial plica-Lateral collateral ligament sprain-Radiohumeral bursitis-Capitellar osteochondritis dissecans-Referred pain from the neck
-Trigger points in the wrist extensors or supinator

That's 12. And therapists argue about this list all the time.

So which one is it? Honestly, they can be really hard to tell apart with lots of overlap. Some even sit together in the same elbow. 😳

Here's what we know for sure…they're all painful.
And pain has to start somewhere.

The lateral epicondyle is a tiny spot where tendon, ligament, joint capsule and periosteum all meet bone. It's packed with nociceptors.

Only nociceptors can send the message.

So why not just treat the pain?

Find what's sensitive. Treat the nociceptors. Then see what shifts.

Can they grip harder? Lift the coffee cup? Shake hands without wincing?
If the answer is yes, you have your direction. No perfect label required.

Want to get really good at finding what's sensitive in the elbow? Our Upper Body course is built for exactly this.

The cancellation…

For most therapists an appointment cancellation is a huge disappointment, but not for a RAPID Therapist!

Why???

Because with RAPID getting people better is our goal.

We don’t want to see you forever.
We don’t need your recurring revenue.
We don’t need you to need us for the rest of your life.

So how do we stay in business?

Because a little known secret in business is that doing your job well and having satisfied patients is far more valuable than having a revolving door of the same people.

Satisfied people trust you more, they promote your business with word of mouth advertising and they help build your reputation for being the top of your field.

So now go out and fix someone! 😉

Apparently, “it still hurts” can travel straight from your client to punch you right in your deepest insecurities. No stops…express service. 😂

We care about helping people. Of course it feels awful when a treatment doesn’t do what we hoped.

But beating ourselves up doesn’t tell us what to do next. Neither does digging harder into the same spot and hoping it finally gets the message.

Ask questions, reassess, and find out what changed and what didn’t. Be willing to change the plan or recognise when someone needs a different kind of help.

With RAPID, we put a lot of emphasis on assessing and rechecking because we want that information guiding our next decision.

We can care deeply about the outcome and still admit, “We need to take another look.”

It’s crazy how you rarely remember some of your most successful cases but you will never forget that one stubborn shoulder that made you reconsider your career choice? 🙋‍♀️

Muscles don’t matter.

Your nervous system sets muscle tone, and pain signals are one of the loudest things it listens to. Research on pain and movement shows that when the body senses a threat, it tightens the area to protect it. That's not the body failing. It's the body doing its job.

Those signals often come from spots packed with pain-sensing nerves, like where muscle and tendon attach to bone. The muscle tightens because it was told to.

Work on it for an hour and it might relax on the table, then tighten right back up. The signal never changed.

Change the signal, and the muscle often follows.

Muscles matter for movement. They just aren't the target.
